Dr. Oren Raphael
Dr. Oren Raphael is a board certified physician with extensive experience caring for medically complex older adults in skilled nursing and post acute care settings. His background spans internal medicine and geriatrics, with a strong focus on clinical judgment, continuity of care, and thoughtful decision making. As Medical Director of Prime Healthcare Solutions, Dr. Raphael sets clinical standards, oversees treatment protocols, and provides physician led oversight across partner facilities. He works closely with nursing and clinical teams to ensure care is delivered consistently and in alignment with regulatory requirements. Dr. Raphael completed his medical training at UCLA, Mount Sinai Hospital, and NYC Health and Hospitals. He is fluent in Spanish and Farsi and is committed to collaborative, patient centered care.
Tomiko Doi-Smith
MSN, FNP-C
Tomiko Doi-Smith is an experienced Family Nurse Practitioner with more than 30 years of clinical experience across acute, rehabilitation, and post acute care environments. Her practice is centered on advanced wound management, limb preservation, and coordinated care for medically complex patients.
She brings hands on expertise in advanced wound care and hyperbaric therapy, along with a strong foundation in evidence based practice and interdisciplinary collaboration. Tomiko works closely with physicians, nursing teams, and facility leadership to support consistent treatment planning and quality outcomes. At Prime Healthcare Solutions, Tomiko delivers direct patient care while supporting clinical consistency across care settings.
Raquel Ochoa
Operations Manager
Raquel Ochoa is an experienced hospice nurse as well as operations manager. She has a background in multiple settings including skilled nursing facilities, primary care, pediatrics, hospice and palliative care . Her background blends hands-on clinical work with operational oversight and team leadership.
She has supported medically complex patient populations by working closely with physicians, nursing teams, and facility leadership to align care delivery with clinical standards and compliance requirements. Raquel brings structure, clarity, and accountability to day-to-day clinical operations.

Anna Lipman
Director of Operations
Anna Lipman is a healthcare operations executive with deep experience building and scaling clinical infrastructure across medical and behavioral health organizations. Her work focuses on creating clear systems that support high-quality care delivery and sustainable growth.
She oversees operational strategy, provider onboarding, workflows, compliance processes, and technology implementation. Anna works closely with physicians and clinical leaders to ensure operational decisions support clinical priorities rather than compete with them.
